NURS FPX 6218 Assessment 2 Assessing Community Health Care Needs
Table of Contents
ToggleStrategic planning for community health assessments enables the identification of problems and needs through structured data analysis. This comprehensive process supports new healthcare initiatives, leading to public health improvements. The Assessment process provides valuable insights into health status, community needs, and concerns, which greatly assist organisations in making informed decisions.
This collected data will help in developing new and better plans that improve community health outcomes (CDC, 2019). This paper assesses the Windshield Survey for Jordan, Minnesota, to evaluate the community needs and conditions. This survey examines the environmental factors and social determinants of health (SDOH). All the findings will be summarized in an environmental analysis and survey report.
General Condition and Public Health Needs
Jordan has a population density of 2,198 people per square mile, and the poverty rate stands at 15.3%. The median age of residents is approximately 31 years to 30.6 years for males and 31.5 years (World Population Review, n.d.). Additionally, over 91.43% of the community speaks English (World Population Review, n.d.).
Community Resources and Facilities:
The windshield survey assesses the healthcare needs of the Jordan community and residents. The results show that Jordan has many community facilities, including the Jordan City Hall (210 1st St. E.), Jordan Middle School, Jordan High School and Jordan Elementary School, the Scott County Library-Jordan, St. Health Services, and Jordan Community Education and Recreation Centre. Seven Churches, including St. John Baptist Church. River Valley Education Centre and Other Facilities like Shopping plazas, parks, restaurants, Fresh Markets, Food Shelves and Residential areas.
There are also many fitness facilities all over the city, like gym stations that track and encourage physical activity. There is also a senior living centre that supports the aged members of the community.
Community Demographics and Areas of Improvement:
While Jordan is a well-developed community, some areas still need a lot of improvement. These improvements help to provide better services to people, like the racial composition:
- White: 90.7%
- Black or African American: 3.26%
- Asian: 0.99%
- Other races are also present
Some key health concerns need to be addressed, like better healthcare services (nutrition support and diagnostic care). (Palinkas & Wong, 2020).
Limitations of the Survey
Comprehensive analysis and accurate statistics are essential for understanding key percentage values. When conducting the survey, it is important to consider potential risks such as theft and crime. According to Jordan’s crime data, a crime occurs every two days, making crime and theft significant safety concerns during the survey process (Crime Grade Data, n.d.).
Environmental Analysis and Health Factors
Environmental analysis helps assess the factors that impact health, both positively and negatively. The key concerns in the community health assessment include:
- Water Quality
- Air Quality
Water Quality in Jordan:
Data from Jordan indicates that the water quality in Jordan is not good (Jordan MN, 2021). The water contains metals like copper and lead, metal oxides and radioactive materials. These materials drastically harm the water quality, which is harmful, especially for infants and people with weakened immune systems. CDC data revealed that contaminated water like this carries germs and chemicals that can cause Cholera and diarrhoea (CDC, 2022).
Air Quality and Its Impact:
The Air Quality in Jordan is not good, with an of 77 (Best Places n.d.). The whole region has 24.2% more pulmonary problems compared to other areas. The air contains particulate matter, which can lead to severe illnesses like Chronic obstructive pulmonary disease (COPD), Asthma and Other lung diseases (Manisalidis et al., 2020).
Socioeconomic Factors and Health Disparities
Low socioeconomic status is linked to:
- Health disparities
- Higher mortality rates
- Health insurance challenges for low-income groups.
NURS FPX 6218 Assessment 2: Assessing Community Health Care Needs Opportunities for Improvement
Water and air problems cause a lot of issues. It is challenging to address and solve them. The main challenges include:
- Worsening health due to poor air and water quality
- Health disparities from economic differences (Data USA, n.d.)
But these challenges also have their solutions, like healthcare providers starting to implement support programs for the community, ensuring equal healthcare access without any discrimination and promoting social justice and cultural competence through seminars (Hatzichristou et al., 2019)
Economic and Business Development:
Some areas in Jordan are vacant. They present an opportunity for equal business and economic growth (Capella University, n.d.). Setting up new businesses can help:
- Improve economic stability
- Create job opportunities
- Enhance living standards
Reducing health disparities due to economic problems by providing equal business opportunities to the community can benefit the people overall.
Social Determinants of Health (SDOH)
Variations in socioeconomic status relate directly to an individual’s access to healthcare resources and other services. The Windshield Survey also highlights the presence of minority groups in Jordan’s community. Different racial groups face unique social, mental, and physical health challenges (Capella University, n.d.), which can largely affect decisions that are related to their health.
Assessing Community Health Care Needs
Socioeconomic inequalities also affect a community’s access to healthcare, food and other living conditions. According to a survey, 15.3% of the people live in poverty and hardship (World Population Review, 2023), and around 12.8% of Jordanians still fall under the poverty line (US Census Bureau, 2022). A growing population with such rates can strain the resources and make access more difficult. Environmental analysis shows that all communities must have access to essential resources to improve public health (Ukaogo et al., 2020).
Summary
The windshield Survey provides all insights for monitoring public health. The data and findings in Table 1 (Appendix) highlight the community conditions and resource availability. Healthcare initiatives and support programs help enhance medical services. These programs make sure that everyone gets equal access to all features regardless of social, economic, or cultural background. Healthcare providers also must promote social justice through seminars and training sessions.
The Key challenges include resistance to change and the need for digital literacy. The survey provided insights into statistical analysis for assessing progress in health initiatives. Both the Windshield survey and environmental analysis will be valuable for future projects by supporting data-driven decision-making.
